Required Documentation

Please fill out this form to express interest in becoming a customer of Curtain Call for Class. In order to process your information, we will need the following:

1. A letter from you on your company letterhead giving us your proper mailing and ship to addresses. Please include the names of all individuals authorized to use your account.
2. A copy of your business license or voided check with your business name and address imprinted on the check.

We request this information to establish the authenticity of each studio that applies to us by doing this we can be sure our prices are only shown to dance studios.
